貳、保險須知

B. Health Insurance

一、團體健康保險 Mandatory Group Health Insurance

•  保險範圍：

•  意外受傷 ( 住院及門診均包括 ) 。

•  重病住院。

•  一般醫療門診不包括在內。

•  此項保險並無特約醫院。

This plan covers hospitalization impatient charges due to accident or serious diseases. Clinics for accidental injuries are included.

參、住宿須知

C. Accommodations

一、宿舍設備 Facility

宿舍備有公用投幣式洗衣機服務。學生須自行負擔 冷氣費 。

Public areas include television rooms and laundry rooms where coin-operated washing machines are available. Residents pay for their own utilities.

D. Facilities

•  圖書館 Library

1. 相關規定

憑學生證進入圖書館。

Students can enter and checked out the library with student ID.

2. 圖書館開放時間：週一至週五上午八時三十分至下午五時，週六至週日及國定假日閉館。

The open hours are Monday through Friday from 8 ： 30~17 ： 00. The Library is closed on Saturday and Sunday , some special holidays , such as Chinese New Year , Tomb Sweeping Day , ext.

•  電子計算機中心 Computer Center

開放時間：週一至週五上午八時至下午十時，週日及假日 ( 除春節、清明節等特殊節日外 ) ，上午八時 30 分至下午四時 30 分。未排定上課或出借的時間，皆開放並提供本校學生自由上機使用。

Computer Center will be available to students when no classes or workshops are held . The open hours are Monday through Friday from 8 ： 00~22 ： 00 , Sunday and Holiday from 8 ： 30~16 ： 30 . (NOTE ： The center is closed on some special holidays , such as Chinese New Year , Tomb Sweeping Day , ext.)

 

•  健康中心 Student Health Center

1. 服務時間：

上課期間：週一至週五上午八時至下午九時三十分，全時段服務。

寒暑假期間：週一至週五上午八時至十二時、下午一時至五時。

Student Health Center will be available on usually ： The open hours are Monday through Friday from8 ： 00~21 ： 30 , service all time.

Student Health Center will be available on winter vacation and summer vacation ： The open hours are Monday through Friday from8 ： 00~12 ： 00 , to13 ： 00~17 ： 00.

一、大葉系所選課流程 The General procedures of course registration are as follows ：

1. 交換生自行至教務處課務組索取課程暨上課時間表，並將欲選之課程名稱送交課務組及註冊組辦理選課相關事宜。

Obtain class schedule in the Curriculum Division. Submit your course selection sheet to the Curriculum Division and the Registration Division.

一、申請本校系所成績單 Transcripts at DAYEH UNIVERSITY

中文成績單需付費 NT$ 二十元、英文成績單需付費 NT$ 五十元

For Chinese version , a fee of NT$20 is required and English version , a fee of NT$50 is required. For more information , please contact the Registration Division.

G. Visa ＆ ARC

  一、辦證程序 Application

1. 至外交部領事事務局辦理『居留簽證』 ( 可加辦多次出入簽證 ) ，約可入境一年。

請攜護照、入學許可信、照片兩張、申請費 2,200 元，至外交部領事事務局：台中市黎明路二段五０三號一樓 ( 「行政院中部聯合服務中心」廉明樓 ) 。
